Does Medicare or insurance cover BridgeCare?
BridgeCare is a private-pay lodging and care coordination service. This means there are no prior authorization delays, no benefit limits, and no 3-day hospital stay requirement. Medicare may cover skilled services that can be delivered in a BridgeCare Suite. Medicare GUIDE program may cover some respite care for eligible patients with dementia.
How quickly can my loved one be admitted?
We coordinate same day and next day admissions.
Who is BridgeCare a good fit for?
BridgeCare is ideal for patients who are medically stable but not yet safe to return home.

Common situations include post-hospital recovery, hospice support, family transition needs, or waiting on services or approvals. We support dementia care and 1:1 supervision, with case-by-case review for complex needs.
What is the minimum stay?
Our minimum rental agreement is for 5 days. Renewable in 5 day increments. Guests may stay as long as needed. We bill weekly (Monday–Sunday).
What is included in the base rate?
The rate includes lodging and supportive care.
Can family visit at any time?
Visiting hours vary by location and situation. Typical hours 8:00a-8:00p. Tours are welcomed and encouraged. Appointments are scheduled by calling 612-393-1505.
What happens if Care Needs Change?
Change in Condition Protocol

If a patient's condition changes beyond the scope of a supportive residential setting, BridgeCare coordinates transfer to the appropriate level of care (e.g., SNF/TCU or hospital evaluation). BridgeCare does not provide emergency or acute medical care. In the event of a medical emergency, emergency services (911) will be activated.

BridgeCare does not provide licensed nursing services; all clinical care is delivered by third-party providers. Our team ensures timely communication and coordinated transitions to maintain patient safety and continuity of care.

Guests may return to BridgeCare in the future if appropriate for their needs.
Can BridgeCare coordinate home health, hospice, or other services?
We coordinate everything for you.
From home care to hospice and support services, we bring the right providers together—and can include your preferred agency when available.

Flexible coordination—beyond placement.
When a BridgeCare Suite isn't required, we can support direct-to-home service coordination. BridgeCare works with skilled, private duty, hospice, and community providers, integrating with preferred agencies when available.
What happens when it's time to leave BridgeCare?
We begin planning early to ensure a smooth transition home—or to the next setting. This includes coordinating home care, equipment, transportation, and referrals as needed. Our goal is for every guest to leave with a clear, well-supported plan.
